Persons or their designated representatives may ask for a copy of their records in a system of records. Privacy Act requests must be made in writing and must also be signed. Send requests for Holloman Air Force Base records to Holloman FOIA, 1321 Tularosa Rd., Holloman AFB, NM 88330-7928. When making a PA request, include either a notarized or unsworn declaration statement in the following format: "I declare under the penalty of perjury that the foregoing is true and correct. Executed on (date). (Signature)" (if outside the United States, use "I declare under the penalty of perjury and under the laws of the United States of America that the foregoing is true and correct. Executed on (date). (Signature)". Requestors need not state why they want access to their records. For requests of records that reside in a System of Records, requestors should be as specific as possible in describing the records being requested. Requestors may also review the Air Force Systems of Records published in the Federal Register or at http://www.defenselink.mil/privacy/notices/usaf. Indicate a willingness to pay fees associated with the processing of your request or, in the alternative, why a waiver of fees may be appropriate. The first 100 pages are free. Copies are $.15 per page, $.25 for fiche. Written signed requests may be received by postal service, facsimile or electronically. All requests must also have a mailing address included since it may not be practical to provide a substantive response electronically.
